Understanding your plan

Common questions, clear answers.

What is a self-funded health plan?

Your employer pays for your health claims directly rather than buying insurance from a carrier. This means your benefits are customized specifically for your organization. Your coverage works the same as any other health plan — you use your ID card, see network providers, and Summit handles the claims.

How do I find an in-network provider?

Log into the member portal and use the provider search tool. You can search by specialty, location, and availability. Your ID card also lists the network name — you can visit that network's website directly to search for providers.

How do I submit a claim?

In most cases, your provider submits claims directly to Summit. If you need to submit a claim yourself (for out-of-network care or international services), download the claim form from the member portal and submit it with your itemized receipt.

What if I need a pre-authorization?

Certain procedures and medications require pre-authorization. Your doctor's office typically handles this, but if you're unsure whether your care requires it, call member services and we'll guide you through the process.

How do I appeal a denied claim?

If a claim is denied, your Explanation of Benefits will include the reason and instructions for filing an appeal. You can also call member services — we'll walk you through the process and help you gather the documentation you need.

What happens when I leave my employer?

You may be eligible to continue your coverage through COBRA. Summit administers COBRA on behalf of your employer and will send you enrollment information. You typically have 60 days to elect continuation coverage.
